We encourage you to register information about yourself in the graduate prospective student database by completing and submitting the form above. Having a record of you and your program of interest will enable the Graduate School and the individual program directors to have a record of your inquiry. Note that this form is NOT an application for admission and that no persons outside the University are given the information. If after reviewing the Graduate School's website you find that you still have questions, you may email us directly or you may contact the graduate director of the academic program in which you are interested in making application.

This report is required of all universities by federal law and discusses crime statistics, procedures for reporting suspicious or criminal activity, security, police authority, crime prevention strategies, university policies on substance abuse and sexual offenses, workplace violence, and fire safety.
